Patients with complex medical problems often have unmet needs, and are at risk of adverse health outcomes following hospital discharge.
Our general practitioners are able to offer these patients continuity of care, and provide information and support to meet the patient’s needs.
Our practices’ provide a diagnostic service for acute medical problems. The doctor will listen to your symptoms carefully, ask some further questions, and then may need to do a physical examination.
At this point a doctor will make a provisional diagnosis, although further tests may be required. Medical processes are not always certain, and at times some degrees of ambiguity may remain.
